Welcome to Your In-Depth Shamanic Aptitude Evaluation

Please take your time with the questions below. When I receive your completed form and your photo, I will review and evaluate your comments and thoughts, and within a few days will do a shamanic journey to determine the full extent of your shamanic aptitude. When the journey has been completed, I will email you to arrange a time for you to call so that we can discuss the results of my evaluation and the content of your journey. The fee for this service is $195, which you will be asked to pay upon submitting your completed form. If you have any questions before beginning to fill out the form, please feel free to email or call me.

You have several payment options. If you choose to pay online, you can pay securely at PayPal, even if you don't have a PayPal account. If you wish to phone in your payment details, please call me toll-free at 1.800.664.0752. I accept Visa, Discover and MasterCard by phone. If you wish to pay by personal check or money order, you can print out your completed payment form and mail it to me, along with payment and a recent photo of yourself, if you don't submit one at the bottom of this page. My mailing address is conveniently printed right on that page.
